The National Transportation Safety Board released a preliminary report of a March 2018 fatal crash in Tempe, Arizona when an Uber self-driving car struck a pedestrian pushing a bicycle. The report states the self-driving system did not recognize the pedestrian at all and only recognized the bicycle just before impact.
